We would like to include the following item in the adoption goodie bag:

* Minimum quantity is 50 per month and must be delivered to the shelter by the 1 st of each month.

**Please note that product samples must be small (e.g. no more that 4-5 small treats) we have several bakeries as sponsors and we don't want overwhelm our adopters.

*** Sponsors may have only one item per month in the goodie bags. Items can be switched out on a monthly basis.

Payment Agreement:

Sponsors must pay in full by the first of each month. Sponsorship payment is for the complete month. Sponsorship can be cancelled at any time. If sponsorship is cancelled after payment for that month, the sponsor will not be reimbursed for the remained of the month. Sponsors can [ay for more than one month at a time.

Sponsor Terms and Conditions:

  1. Sponsors must submit payment by the first of every month. There is a 5 day grace period.

  2. Sponsors must submit an ad to fit in a 5x7 format for the signage on runs and cat room walls and electronic version of logo (jpg file) for weekly flyer (if you you're your logo on the flyer) with sponsor application. E-mail to mail@maxfund.org.

  3. All sponsor ads and logos are subject to MaxFund approval. We are a family facility!

  4. Sponsors understand that this is a fundraiser for the MaxFund Animal Adoption Center , a 501(c)(3) non-profit organization. Sponsors will not be promised product or service exclusivity.

  5. All sponsor fees are 100% tax deductible.

  6. All new sponsors must have application and payment submitted by the 10 th of the month to be recognized in the following month's newsletter.
